What needs to change?
The problem is not the design of Bus Stops, for years people managed to anarchicly organise themselves into queue's with only a post for help. London Transport could redesign shelters to form a queue, although it is exactly this mentality which has brought us to this sorry state of affairs in the first place.
We do not need more control, control is what has caused this breakdown in common sense and decency!
We need to find a way of bringing out Joe Public's and Johnny Foreigners sense of anarchy. If people could be once again made responsible for their own actions, and regain a sense of fair play then the re-emergence of the Bus Queue would be a natural progression.
What are the problems?
1 - There is too much social and architectural control on people in London, especially within the public transport system. People are so used to being shepherded around that as soon as they reach a free space (such as a bus stop) they are incapable of self organising.
2- The English reserve, people are too shy to protest when an unruly rogue pushes in.
3- Bus Driver Bus Stopping, Bus drivers do not stop at Bus stops! Due to the breakdown from anarchy to chaos people cause such a scrum that drivers tend to stop before or after the stop making people walk to the bus and create a temporary race/queue.
What can Transport For London Do?
As we have said this is not a design but a social problem, however a simple change that TFL could make is:

TFL need to install a different Bus Stop post for every number bus rather than several buses sharing the same post and people not knowing what stop they are waiting for. |
